Registration Procedure
New/Transferee Students
Requirements:
-
Original copy of Form 138 (High School Card)
-
Original copy of Good Moral Character Certification
-
Barangay Certification (for Makati Residents only)
-
Certificate that the Form 137-A has not been sent to any College/University (for student who did not immediately enrol after graduation from high school)
-
1 pc 1x1 ID picture colored
-
Mailing Envelope (Long w/ stamp)
-
1 Long Folder/ 1 Long Brown Envelope
-
Admission & Advising Slip (issued by UMAK Admission Office)
-
Medical Clearance stamped properly by UMAK Medical/Dental Clinic on the advising slip of the student.

Transfer Credential/Honorable Dismissal & Transcript of Records (for Transferee only)
Procedure:

The student fills up the Advising Slip with the official class schedule of his prefered section under the program where he qualifies. (The Advising Slip should bear the UMAK Medical Clearance stamp)

The student presents his Advising Slip and other requirements to the advising/registration officer of the college at designated area for approval of the course load.

The student proceeds to the PE/NSTP-CWTS or ROTC Registration area to enlist in the said course/s. The said advising should be stamped properly.

The student proceeds to the designated area for PIA,TMC|SSG and School Organization/club for necessary membership fees. (The Advising slip should be stamped properly).

The student proceeds to the Accounting Office for the assesment.

The student proceeds to the Office of the University Registrar (Mezzanine, New Adminsitration Building) for proper encoding and submission of all the requirements.
6.1. Only students who accomplished Procedures No. 1 to 5 shall be encoded.

The student pays the necessary token fees, Guidance Fee and ID fee to the Cashier at designated area. The cashier accepts payment, issues official Receipt and affixes his signature.

The student proceeds to the Office of the Registrar and presents his approved Advising Slip and Official Receipt for COR printing and ID picture taking.

The student receives his/her final copy of COR.
